PHYSICIAN REFERRAL
From a hospital, skilled nursing facility, or physician's office.
PATIENT SCREENING
The home health agency will review the referral to determine if the patient meets eligibility requirements.
INITIAL CONTACT
The Home Health agency will contact the patient to discuss their needs and schedule an initial assessment.
INITIAL ASSESSMENT
A skilled nurse will visit the patient in their home to conduct a thorough assessment, including a medical history, physical examination, functional evaluation, medication review, and environmental assessment.
MEDICAL NECESSITY DETERMINATION
Evaluate the patient's needs for home health services, such as skilled nursing, PT, OT, ST, MSW, CHHA, etc.
CARE PLAN DEVELOPMENT
Based on the assessment, a comprehensive care plan outlining specific services, interventions, and goals will be created.
SERVICE INITIATION
The home health agency will begin delivering the planned services.
