Step 4Home Visit

INITIAL ASSESSMENT

A skilled nurse will visit the patient in their home to conduct a thorough assessment, including a medical history, physical examination, functional evaluation, medication review, and environmental assessment.

Step 5Care Plan

MEDICAL NECESSITY DETERMINATION

Evaluate the patient's needs for home health services, such as skilled nursing, PT, OT, ST, MSW, CHHA, etc.

Step 6Implementation

CARE PLAN DEVELOPMENT

Based on the assessment, a comprehensive care plan outlining specific services, interventions, and goals will be created.

Step 7Ongoing Support

SERVICE INITIATION

The home health agency will begin delivering the planned services.

Is home healthcare free? How do I confirm if I'm eligible for Medicare-covered home health services?

If you are 65 or older with Medicare Part A and Part B (Red-Blue Card), you can contact us directly to help determine your eligibility. If you qualify, Medicare typically covers compliant medical/rehabilitation services in home healthcare in full, at no cost to you. What services does home healthcare include?

Common services include: registered nurse care, physical therapy, occupational therapy, speech therapy, home health aide (assistance with daily activities like dressing and bathing), wound care, medication management, condition education, and chronic disease management. Some agencies also provide diagnostic services such as mobile blood draws/labs and X-rays.

What happens during the first visit?

• Professionals conduct a detailed assessment. • A care plan is developed with you, your family, and your doctor. • Service frequency, types, and personnel arrangements are determined.

How frequent are home healthcare visits? Can they be adjusted?

• Visit frequency and service types are based on your care plan and doctor's recommendations. They may be several times a week or intermittent. • If your condition improves or changes, you can discuss adjustments to the plan with your care team and doctor.

What's the difference between Home Healthcare, Hospice, and Home Care?

• Home Health Care: Medical/rehabilitation in nature, provided by licensed professionals, aimed at restoring, maintaining, or managing health. • Hospice: For terminal illnesses, focusing on symptom relief and quality of life rather than cure. • Home Care / Personal Care: Mainly assists with daily living activities such as bathing, dressing, cooking, and cleaning, without medical/rehabilitation professional operations.

Who provides the services? How is their professionalism ensured?

• Personnel visiting your home typically include registered nurses (RN), licensed practical/vocational nurses (LPN/LVN), physical/occupational/speech therapists, home health aides, medical social workers, etc. • These personnel hold licenses or certificates issued by national or state authorities and undergo training and background checks. Agencies generally have supervisory mechanisms to ensure service quality.

How long will services continue? When can I stop?

• This is determined by your doctor and care team based on your recovery progress and plan. If you recover well or achieve medical/rehabilitation goals, service frequency can be reduced or terminated. • Patients can also request changes or discontinuation of services and should communicate with their doctor and agency.
